Appointments
Your time is important to us. When booking an appointment online, please select two date/time slots. Though only one is required to complete our online form, it gives us a fallback. Appointments requested online are tentative until we contact you back to solidify your appointment date/time.
Sometimes, life presents circumstances that require you to reschedule an appointment. If you must reschedule or cancel an appointment, please call our office 1 business day before your scheduled appointment to avoid charges.

Memberships
Adult or Child
We offer an annual adult membership to our practice that includes: 2 basic cleanings, 2 exams, and 10% off additional treatments/procedures. The annual cost for an Adult Membership is $200. The annual cost of a Child Membership is $125. Please contact our office to arrange payment and receive your membership card.
New Patient Forms
Welcome to Big Smile Dentistry! We look forward to servicing your dental health needs. You can download the new patient forms, print them out, and complete the filling out process prior to your appointment if you wish.
Financial & Payment Options
We accept Visa, MasterCard, Discover, PayPal, or American Express for payment regarding office visits, treatments and procedures.
We also recommend CareCredit® for low-interest installment payments especially when the patient must have necessary higher cost care such as a root canal or dental crowns.
Insurance
All patients are responsible for understanding their plan/coverage. The patient is responsible for filing their claim independently and must pay upfront at the time of the office visit.
